What is in the AMAS® test shipping kit?
The AMAS® test, collection kit contains special tubes for collecting the blood for the AMAS®TEST, instructions for handling of the serum and its shipping, a requisition form and literature regarding the AMAS® test.

What is the price of the shipping kit?
The shipping kit is free.

About payments

What is the price of the AMAS® test?
One hundred sixty five dollars ($165.00) USD is paid to Oncolab. The costs of drawing the blood, separating the serum and shipping the serum in dry ice by overnight FEDEX morning delivery are not paid by Oncolab.

What forms of payment does Oncolab accept for the AMAS® test?
Oncolab accepts checks, money orders, and credit cards (Visa, Mastercard, and American Express). Medicare pays Oncolab for tests for those on Medicare.

Does Oncolab accept any forms of insurance?
Oncolab accept only Medicare for the AMAS® test.

Do insurance companies reimburse paitents for the AMAS® test?
Oncolab does not accept any insurance other than Medicare. Individual customers must check with their insurance company to determine if it reimburses for the AMAS® test. It helps to tell the insurance company that Medicare pays Oncolab for the test. If a customer wishes to receive a receipt for payment, please request the receipt on the Requisition Form by checking the box “Receipt Requested”.

About Medicare

How does an individual who has Medicare coverage order the AMAS® test?
Individuals who have Medicare coverage should not send payment for the test. Customers need simply to:
1. enclose a copy of both sides of the Medicare card to the Requisition Form;
2. include the physician’s UPIN number on the Requisition Form.

Oncolab will bill Medicare.

About shipping

How is the AMAS®TEST Serum sample shipped to Oncolab?
The AMAS® test Requisition Form lists the instructions to prepare and ship the serum to Oncolab.

Can regular ice be used to ship the AMAS® test serum sample?
No. Regular ice will not keep the serum frozen cold enough, and it will melt en route. Only dry ice will keep the sample frozen.

Can an ice pack be used to ship the AMAS® test serum sample?
No. Only dry ice will keep the serum specimens at the proper frozen temperature until they reach Oncolab.

Can the AMAS® test serum sample be frozen in liquid Nitrogen and then shipped on ice packs or regular ice?

No.


About dry ice

Where can dry ice be found?
Large labs that are used to shipping serum specimens to other laboratories carry their own dry ice. Local supermarkets usually carry dry ice. Also consult the yellow pages for dry ice. You may also try online at www.dryicedirectory.com.

About test results

What is the turn-around time for the results on the AMAS® test?
Results are sent out within two to three business days after Oncolab receive the serum sample. The results are sent to the doctor only.

If I have not received the result for my AMAS® test, what should I do?
Call your doctor. If your doctor does not have the results please call Oncolab at (800) 9CA-TEST.

How do I find a lab or doctor in my area who is familiar with the test?
Oncolab now has expanded nationwide directories of physicians who are familiar with the test, and laboratories with experience drawing and shipping samples for the test. For assistance locating a physician or lab in your area, please call us at 617-536-0850, 9AM-5PM EST, Monday through Friday.
